We need to upgrade our OS on our current SQ 9.4 server running on Windows 2012. This will require us to stand up a new server running Windows 2019 and move SQ to that server. Can you provide any documentation or guidance on the best process to migrate? The database is on a separate host and won’t be moved. I appreciate any help provided. Thank you!

Your version is past EOL. You should upgrade to either the latest version or the current LTS at your earliest convenience. Your upgrade path is:
9.4 → 9.9 → 10.1 (last step optional)

Regarding your question, the answer is much the same as for Changing your edition. I.e. just configure your (ideally, upgraded) edition on the new server to point to your old DB and fire it up.
